The Opportunity

Page Healthcare was introduced to the CEO of Hampton Roads Health Center, Mrs. Barbera Willis, in 2017 through the economic development office in Portsmouth, Virginia. The health center was weighing the benefits of renovating the existing clinic or whether relocating the main clinic to a new site made more sense. The current clinic building was very old and at the end of its usefulness. Page Healthcare Development was engaged to assist Hampton Roads Community Health through the infrastructure evaluation of the existing building and to determine if the building would move where it should go. The client was up against a tight timeframe because they received a $1M infrastructure grant from HRSA, and they needed to explain the circumstances of relocating the grant to a new property.

The existing clinic building was not in an ideal location, nor could it provide the client with the required space to add additional services. In addition, the building had moisture concerns, offered very limited expandability from a structural perspective, and needed more parking for the expansion. We proposed that Hampton Roads Community Health develop a new outpatient health clinic, off-balance-sheet, using a synthetic lease arrangement that would allow the health clinic to get what it needed without putting up all the required capital.

The goal was to find a suitable replacement location for a new clinic. We located some potential sites and determined which would be best suited to receive approximately 25,000 – 30,000 square feet of outpatient medical space. We also assisted the health center with preparing the necessary business case to raise the additional capital via tax incentives, equity capital, and senior debt. The engagement included a thorough understanding of the growth strategy, site investigative studies, construction testing, environmental reviews, program validation, test-fit renderings, conceptual layout, determining the probable construction cost, and financial modeling.

Page Healthcare assisted the leadership at Hampton Roads Community Health through negotiations with the Health Resources Services Administration to successfully retain the $1,000,000 federal grant and obtain land control of city-owned property valued at $2,000,000. We assisted the health center through the predevelopment process. We developed a successful strategy to complete a new $7,500,000 clinic facility that included primary care, pediatric care, women’s health, men’s health, dental services, behavioral health and counseling services, pharmacy suite, and administration space.

The Opportunity

Page Healthcare was referred to the CEO of Medical Associates Plus to evaluate the existing clinic and determine if an expansion project could be done on the adjoining church property. The health center was entirely out of space and needed additional capacity for medical services, a pharmacy, counseling space, administrative offices, and a dental clinic. The existing clinic operated out of a triple-wide trailer with limited capacity. Parking was shared with the church on the adjacent property, but the clinic did not own most of the land. Page Healthcare Development was engaged to assist Medical Associates Plus through the infrastructure evaluation of the existing building and to determine if the clinic needed to relocate to a new site or build further on the current land.

The existing clinic location was not ideal. It needed to be repositioned for greater visibility and expanded to provide the client with the required space to add additional services. In addition, the health center did not own the land, and to accommodate development on site, the health center would need to negotiate a land transaction with the church. The clinic would use much of the parking during regular business hours, and the church would use it during evening events, Saturdays, and Sundays for Worship Services. We proposed that Medical Associates Plus develop a new outpatient health clinic on campus via a long-term ground lease with the church. We developed a construction operations strategy to construct the building in phases to accommodate relocating the clinic into the new building, then shift construction operations to the old building, demolish the trailer, and build additional parking in its location. All of this needed to be done while keeping the health center operational during the construction of the new building and coordinating operations such that we did not impact church parking and functions in any manner.

Page Healthcare assisted the leadership at Medical Associates Plus through preconstruction negotiations with the Health Resources Services Administration to clear the use of $4,750,000 in federal grants to cover the construction cost and purchase the necessary equipment for the clinic. We managed the design process and built a state-of-the-art facility that included the following services: primary care, pediatrics, women’s health, men’s health, dental, behavioral health, and counseling services, a pharmacy suite, and adequate administration space. We expanded the parking to accommodate the demand for an 18,500-square-foot health center.

The Opportunity

Page Healthcare was introduced to the former CEO of J.C. Lewis Health Center, Mr. Brandon Wilson, in 2018. J.C. Lewis Health Center was in the process of determining where to relocate the main clinic site. The existing clinic lease was scheduled to expire in 16 months, so the health center needed to make decisions quickly to complete a study, finish the design, and renovate a building within the allowable schedule. J.C. Lewis Health acquired a vacant retail location on the campus edge of the Oglethorpe Mall in Savannah, Ga. The building once served as the Thompson Trophy Shop, and it was initially constructed as a flex retail/warehouse space with a few business offices. Page Healthcare was engaged after the building acquisition with the assistance of the original architect. Upon joining the team, we requested complete documentation on how the decision was reached to acquire the facility, and we also asked for documentation on the investigative study to determine that the building could be converted to accommodate an outpatient health program.

Our role included overall program management for a replacement facility where the target location was an existing building constructed in the early 80’s. The goal was to convert the existing building into an outpatient clinic, an adaptive reuse project. The project consisted of a single-story facility with approximately 14,051 sf. The engagement included a thorough understanding of the growth strategy, site investigative studies, construction testing, environmental review, programming validation, test-fit renderings, conceptual layout, design, probable construction cost development, and capital stack development. From the start of the engagement, we guided the client through reprogramming and a design strategy that included interior and exterior elements. With the understanding that the client’s financial resources were limited, we advised the client that the project be designed with expansion in mind, and we created two separate phases to achieve the maximum renovations with the available funds. The challenge was that the building was designed to be something other than an outpatient health center, so the plan required an intense renovation of the site utilities serving the building and structural upgrades to the superstructure of the building. We had to match the available budget with the probable program and scope to accommodate at least twelve exam rooms and four dental operatories. We had a phase two plan that included six more exams and two additional dental operatories.

Page Healthcare was able to lean on its experience in the adaptive reuse environment and created a strategy to maximize both the budget and the available space to improve utilities, strengthen the frame of the building, and increase the power capacity. With the shell building improvements, we could accommodate the following scope of services: Primary Care, Optometry, Pharmacy, Dentistry, Behavioral Health, Lab, and space for Administration, including auxiliary space for storage needs and building utilities. While we were under construction, J.C. Lewis Health received additional federal funding via a grant from HRSA to initiate phase two, and we opened the center with the whole program available to see more patients in the Savannah Community.

Revive Orthopedics, started the planning process for a new surgery center in 2020. Revive acquired an existing premanufactured building in Lithia Springs, Ga that once served as a mixed-use office building. The challenge was that the existing building was in moderate condition, but it was not designed to the standards needed to accommodate an orthopedic surgery center. The building had already been purchased, contracts for architectural design and engineering solidified, and construction procured through a design-bid-build delivery process before Page Healthcare got involved with the project.

Page Healthcare’s Role:

Page Healthcare was engaged mid-stream of the project to provide project management, design management/document coordination, equipment planning and overall coordination between the entire project team. Our responsibilities included:

Change Order Management: Evaluate potential changes and identify the responsible party, considering factors like design coordination, existing conditions, and the premium associated with making the change.

Optimize Construction Scheduling: Incorporated two-week look ahead schedules to keep the end-user up to speed on construction operations that might impact clinical operations. The ASC sits between two active patient clinics.

FF&E Constructability & Planning: Assumed management control over owner furnished and contractor installed equipment planning. We also coordinated owner furnished owner installed surgical equipment with the equipment vendors.

Pay Application Review & Approval: Review progress on a bi-weekly basis to ensure accuracy with percentage completion included within the billing.

Project Closeout: Worked with the contractor on inspections with city county and state fire marshal. Assisted with equipment start up and training and coordinated closeout manuals for the owner’s facility team.

Project Outcomes:

In the end, we were able to streamline communication, improve project coordination, and reduce the need for unnecessary change orders. The project resulted in a quality-built facility that is properly equipped to better meet the needs of the community. Revive Orthopedic surgery center primarily focuses on procedures related to the musculoskeletal system, encompassing a wide range of treatments. These typically include minimally invasive arthroscopic procedures for conditions like torn meniscus or rotator cuff tears, joint replacement surgeries (hips, knees, shoulders), fracture repairs, spinal surgeries, and procedures for conditions affecting the hands, wrists, feet, and ankles.

The Community Health Hub, located in a 16,320-square-foot building near downtown Franklin, will provide comprehensive medical, dental, and behavioral health care, alongside domestic violence counseling, legal services, and community advocacy – all offered on a bilingual basis. This effort is directly inspired by unmet needs identified during community outreach, particularly exacerbated by the pandemic.
